چکیده

Background A novel technology has been developed for ultrafast (UF) singlephoton emission computed tomography (SPECT) myocardial perfusion imaging by employing a pin-hole collimation design and multiple cadmium zinc telluride crystal arrays. The purpose of this study was to compare myocardial perfusion imaging obtained by UF-SPECT with standard (S) SPECT in patients with known or suspected coronary artery disease. Methods and Results A total of 34 patients underwent single-day TcTetrofosmin stress/rest myocardial perfusion imaging. UF-SPECT was performed 10 min before S-SPECT. Images were qualitatively analyzed, the summed stress score (SSS) and summed rest score (SRS) were calculated. The segmental tracer uptake value (percentage of maximum myocardial uptake) was also quantified for both UFand S-SPECT. When only 29/34 patients with significant coronary lesions were analyzed, SSS was 10.1 ± 4.4 versus 6.4 ± 2.9, respectively for UFand S-SPECT (p=0.002). Qualitative and quantitative per-patient analysis showed similar results in detection of coronary artery disease for UFand S-SPECT. In contrast, per-vessel analysis demonstrated higher regional sensitivity of UFversus S-SPECT. UF-SPECT showed higher sensitivity in detecting multivessel disease (p = 0.003, versus SSPECT). Conclusions This pilot study confirms that UF-SPECT provides high quality fast myocardial perfusion imaging, and suggests that it may allow a more accurate evaluation of both extent and severity of myocardial ischemia in patients with coronary artery disease.

Myocardial perfusion scan accuracy in detection of coronary artery disease - Comparison with exercise stress test [Persian]

Introduction: In patients with coronary artery disease (CAD) noninvasive evaluation for detection of ischemia is important to avoid invasive interventions like angiography. Exercise stress test is conventionally the first study used in evaluation of CAD. Considering the noninvasive nature of the myocardial perfusion scan, we decided to compare its accuracy with stress test. Methods: Patie...

Performance of myocardial perfusion imaging using multi-focus fan beam collimator with resolution recovery reconstruction in a comparison with conventional SPECT

Objective: IQSPECT is an advanced high-speed SPECT modality for performing myocardial perfusion imaging (MPI), which uses a multi-focus fan beam collimator with resolution recovery reconstruction. The aim of this study was to compare IQSPECT compared with conventional SPECT interms of performance based on standard clinical protocols. In addition, we examined the concordance between convention...

Relationship between the location of the most severe myocardial perfusion defects, the most severe coronary artery stenosis and the site of subsequent myocardial infarction [Persian]

This study evaluated the relationship between the location of the most severe coronary artery stenosis and the subsequent acute myocardial infarction (AMI). Methods: Of 1590 patients who were admitted with a diagnosis of AMI, we identified 44 patients who had undergone previous myocardial perfusion SPECT. Thirty-one of these patients also had previous coronary angiography. The relationshi...

Gated 99mTc-MIBI myocardial perfusion SPECT in patients with right bundle branch block but without evidence of coronary artery disease

Introduction: We aimed to investigate the effect of right bundle branch block (RBBB) on perfusion and functional parameters in dipyridamole stress/rest Tc99m-MIBI gated myocardial perfusion SPECT (GSPECT) which may be helpful in interpretation of myocardial perfusion imaging. Methods: We studied 73 patients with low pre-test likelihood of coronary artery disease in two groups:  38 patients wit...
